Using the McCarthy Test to detect femoroacetabular impingement and other articular pathologies

Purpose: This test is used to assess for articular pathology, particularly femoroacetabular impingement (FAI) and minor lesions of the labrum.

Type of Test: This is a passive joint movement test.

Procedure: Standing to the side of the plinth with your client supine, use one hand to cup their heel. Place your other hand just above their knee and gently take the hip into full flexion (figure 2.8a). Next, slowly extend the knee, lowering the leg whilst simultaneously putting the limb into external rotation (figure 2.8b).

Findings: The test is positive if it elicits a click and/or reproduces the client’s pain. If the maneuver does not elicit a positive response, repeat the test, this time putting the limb into internal rotation.
