Dix Hallpike Test

Used to assess: Paroxysmal vertigo and benign positional paroxysmal vertigo

Patient position: Seated on a treatment table with extended legs

Clinician position: Standing at the end of the table

Clinician’s stabilizing hand position: N/A

Clinician’s test hand position: One hand on either side of patient’s head

Action performed: The clinician rotates the head 45° toward the ear to be tested (a). The patient is then rapidly laid down with their head hanging off the end of the table, resulting in about 20° of cervical extension with the 45° of rotation maintained (b). The clinician observes the patient’s eyes for nystagmus.

Positive result: Nystagmus

Accuracy: SN = .79 SP = .75 +LR = 3.59 −LR = .14
